Monthly Apple Watch Giveaway — Official Rules

1. Sponsor

The OMNI Longevity Monthly Apple Watch Giveaway (the “Giveaway”) is sponsored and administered by OMNI Health LLC, 8 The Green STE A, Dover, DE 19901 (the “Sponsor”). The Giveaway is in no way sponsored, endorsed, administered by, or associated with Apple Inc. Apple is not a participant in or sponsor of this promotion.

2. Eligibility

The Giveaway is open only to legal residents of the United States (49 states and the District of Columbia, excluding Rhode Island) who are at least eighteen (18) years old at the time of entry. Employees, officers, and directors of the Sponsor and its parent companies, subsidiaries, affiliates, and agencies, as well as members of their immediate families and households, are not eligible. The Giveaway is void in Rhode Island and in any jurisdiction where prohibited or restricted by law.

3. Giveaway Period

The Giveaway is conducted in consecutive monthly periods. Each monthly period begins at 12:00:00 a.m. Eastern Time on the first day of the calendar month and ends at 11:59:59 p.m. Eastern Time on the last day of that month (each a “Giveaway Period”). The first Giveaway Period begins on September 1, 2026, and the Giveaway automatically ends after twelve (12) consecutive monthly Giveaway Periods unless ended earlier or extended by the Sponsor. Entries do not carry over from one Giveaway Period to the next.

4. How to Enter

There are two ways to receive one (1) entry into the Giveaway during a Giveaway Period. Each entrant may receive a maximum of one (1) entry per Giveaway Period regardless of entry method.

• Entry with purchase: Place your first-ever subscription order for any OMNI Longevity product at omnicreatine.com during the Giveaway Period. You will automatically receive one (1) entry. Customers who have previously held a subscription are not eligible to enter through this method.
• Free entry (no purchase necessary): Complete and submit the free entry form at with your full name, email address, and state of residence during the Giveaway Period. Free entries are treated equally with purchase entries in all respects.

Duplicate entries received from the same person, email address, or household within a single Giveaway Period are void. Entries that are incomplete, inaccurate, late, or otherwise not in compliance with these Official Rules are void. Proof of submission of an entry does not constitute proof of receipt by the Sponsor.

Entries generated by script, bot, or any automated means are void. The Sponsor’s records are the official time-keeping device for the Giveaway.

5. Winner Selection and Odds

One (1) potential winner will be selected in a random drawing from all eligible entries received during the applicable Giveaway Period. The drawing will take place within the first week of the calendar month following the end of that Giveaway Period. Odds of winning depend on the total number of eligible entries received during the Giveaway Period.

6. Winner Notification and Verification

The potential winner will be notified by email at the address associated with the entry. The potential winner must respond within five (5) days of the notification and provide any information reasonably requested by the Sponsor, which may include confirmation of eligibility and execution of releases. If a potential winner does not respond within this period, cannot be verified, or is found ineligible, the prize will be forfeited and, at the Sponsor’s discretion, awarded to an alternate winner selected by random drawing (up to two alternate drawings). Please allow approximately six to eight (6–8) weeks for prize fulfillment following verification.

7. Prize

One (1) prize is awarded per Giveaway Period: one (1) Apple Watch Series 11 or, if that prize becomes unavailable, a comparable smartwatch of equal or greater value, as determined by the Sponsor. Approximate retail value: $399.00. The prize is non-transferable, may not be exchanged, and may not be redeemed for cash except where required by law. A limit of one (1) prize per person, household, and email address applies per Giveaway Period. The winner is solely responsible for all taxes and any other costs and expenses associated with prize receipt and use. No substitution of prize is permitted except by the Sponsor, which reserves the right to substitute a prize of equal or greater value.

8. General Conditions

By entering, entrants agree to be bound by these Official Rules and by the decisions of the Sponsor, which are final and binding in all matters relating to the Giveaway. The Sponsor reserves the right to disqualify any individual it finds to be tampering with the entry process or the operation of the Giveaway, acting in violation of these Official Rules, or acting in an unsportsmanlike or disruptive manner. The Sponsor reserves the right to cancel, suspend, or modify the Giveaway if fraud, technical failure, or any other factor beyond the Sponsor’s reasonable control impairs the integrity or proper functioning of the Giveaway. If the Giveaway is cancelled, suspended, or modified for such a reason, the Sponsor may, in its sole discretion, select the winner by random drawing from among all eligible, non-suspect entries received before the action requiring such cancellation, suspension, or modification.

9. Release and Limitation of Liability

By entering, entrants release and hold harmless the Sponsor and its parent companies, subsidiaries, affiliates, and their respective officers, directors, employees, and agents from and against any claim or cause of action arising out of participation in the Giveaway or receipt or use of the prize, except in the case of gross negligence, fraud, or willful misconduct. The Sponsor is not responsible for technical malfunctions, lost or unavailable network connections, or late, lost, incomplete, garbled, or misdirected entries or communications.

10. Privacy

Information submitted with an entry is collected and used in accordance with the Sponsor’s Privacy Policy, available at omnicreatine.com. Entrants who opt in may receive promotional communications about OMNI Longevity products and services and may unsubscribe at any time. Consent to marketing is not a condition of entry. Entrant information is not shared with third parties except as described in the Privacy Policy or as required by law.

11. Disputes and Governing Law

Except where prohibited, each entrant agrees that any dispute arising out of or connected with the Giveaway shall be resolved individually, without resort to any form of class action, and exclusively by the appropriate state or federal courts located in Travis County, Texas. All issues concerning these Official Rules or the Giveaway are governed by the laws of the State of Texas, without giving effect to any conflict-of-law rules.

12. Severability

If any provision of these Official Rules is held to be invalid or unenforceable, that provision will be enforced to the maximum extent permissible, and the remaining provisions will remain in full force and effect.

13. Winner List

To request the name of a winner, send a self-addressed stamped envelope to OMNI Health LLC, 8 The Green STE A, Dover, DE 19901, postmarked within ninety (90) days after the end of the applicable Giveaway Period, or email support@omnicreatine.com. Vermont residents may omit return postage.
